- Appeal Form for Class Drop (PDF)
Use this form if you missed the refund deadline or the withdrawal deadline due to extenuating circumstances. The completed form must be received in the Registrar's Office by the last day of the semester in which the course is scheduled.
- Appeal Form for Grading Option Policy (PDF)
Use this form if you enrolled in a course for the pass/no pass credit option and you wish to change the letter grading option (or vice versa) and failed to do so within the registration period.

To earn your degree, you need to complete a graduation application. A form is required for each degree you will earn at GSU. A $50 non-refundable processing fee will be charged for each graduation application. If you will finish your degree requirements by the end of the spring or summer should complete the application and turn it into the Registrar's Office, located in C1300, by Friday, February 7, 2014. Students who are unsure if they are eligible to graduate should check with their advisor before submitting a graduation application. The graduation application for spring or summer 2014 will be available in early December.
- Cancellation of Graduation Application (PDF)
This form is used if a student wishes to cancel a graduation application. A $50 non-refundable fee is still charged for the original graduation application.
- Change of Student Information (Personal Data) (PDF)
Use this form if you have changed your name, address, telephone number or to add report your social security number to your student record.
- Change of Major/Status Form (PDF)
Use this form only if you are a new, continuing or undeclared student and you wish to change your major, add a second major, change a concentration and/or add a certificate to your program. If you are a non-degree seeking student, please contact the Admissions Office to apply to a program.
- Continuing Registration Request Form (PDF)
Use this form if you are requesting continuing registration a course. Approval for continuing registration must be granted by the course instructor and by the department or division chairperson. NOTE: Continuing registration is an option for only a small number of graduate level program capstone courses; it is not an option for the vast majority of University courses.
- Continuing Student Reactivation Form (PDF)
Use this form if you are a degree-seeking student and you have not registered for more than 3 consecutive semesters (but not exceeding 15 consecutive semesters), and you are not on academic suspension.
- Extended Incomplete Request Form (PDF)
Use this form if you are requesting an extension of an Incomplete (“I”) or continuing registration status for a course. You must have extraordinary and compelling reasons why an additional extension is needed to complete unfinished course requirements. Authorization for an extension must be granted by the instructor with approval from the college dean.
- Invitation Letter for International Graduates (PDF)
This form is used for International students to send to the United States Consulate for guests to attend the Commencement ceremony.
- Request For Official Grade Report (PDF)
Grades are available on line and are not mailed except upon specific request. Use this form to request to have grades mailed.
- Request to Prevent Disclosure of Directory Information (PDF)
Under the provisions of the Family Educational Rights and Privacy Act of 1974 GSU may release, at the discretion of the university, directory information pertaining to individual students without their written consent. However, the Act gives currently enrolled students the right to withhold the disclosure of directory information. Use this form if you do not want the university to release directory information.
