Sample Letter of Teaching Verification (This letter must be submitted on original Superintendent's letterhead.  Have the School District place in a envelop and sign the back.)

Sample Letter for Chief School Business (This letter must be submitted on original Superintendent's letterhead.  Have the School District place in a envelop and sign the back.)

Sample Letter for Higher Education Verification (This letter must be submitted on original college/university letterhead.  Have the college/university official place in a envelop and sign the back.)

In a separate envelope, submit the University Graduate Application which can be printed with one set of official transcripts from all colleges and universities previously attended and the $50.00 application fee to:
