Directions: My office is located in Room C3350. To reach my office, park in Lot D/West 1 or Lot D / East 1, and head for the front Main Entrance (clearly marked) of the University. Walk through the lobby area (past Student Central) and go to the elevator bank. Take the elevator upstairs to the 3rd floor. Exit right off the elevator, and follow the hallway down and around until you reach the Department of Nursing, where our receptionist will greet you. My office is located just inside the main Nursing office, to your right. I'll be happy to see you!

Below are links to all forms required once you are admitted to the RN to BSN Completion Program. All of the other forms (Incident Form, Informed Consent, Potential Disease Exposure Form, Refusal of Hepatitis B Form, Student Handbook Sign-Off Sheet, Universal Precautions Training Acknowledgement, and Student Health Form) are required along with a copy of your current RN license, malpractice insurance, personal insurance, and CPR card.
