Use this form if you missed the refund deadline or the withdrawal deadline due to extenuating circumstances. The completed form must be received in the Registrar's Office by the last day of the semester in which the course is scheduled.

Use this form if you have changed your name, address, or telephone number. If you are an international student and have received your U.S. social security card, use this form to report your social security number.
- Change of Major/Status Form (PDF)
Use this form only if you are a new, continuing or undeclared student and you wish to change your major, add a second major, change a concentration and/or add a certificate to your program. If you are a non-degree seeking student, please contact the Admissions Office to apply to a program.
- Continuing Registration Request Form (PDF)
Use this form if you are requesting continuing registration a course. Approval for continuing registration must be granted by the course instructor and by the department or division chairperson. NOTE: Continuing registration is an option for only a small number of graduate level program capstone courses; it is not an option for the vast majority of University courses.
- Continuing Student Reactivation Form (PDF)
Use this form if you are a degree-seeking student and you have not registered for more than 3 consecutive semesters (but not exceeding 15 consecutive semesters), and you are not on academic suspension.
- Enrollment/Degree Verification Request Form (PDF)
Use this form if you are requesting verification of enrollment or a degree.
- Extended Incomplete Request Form (PDF)
Use this form if you are requesting an extension of an Incomplete (“I”) or continuing registration status for a course. You must have extraordinary and compelling reasons why an additional extension is needed to complete unfinished course requirements. Authorization for an extension must be granted by the instructor with approval from the college dean.

This form is used for International students to send to the United States Consulate for guests to attend the Commencement ceremony.

- Request to Prevent Disclosure of Directory Information (PDF)
Under the provisions of the Family Educational Rights and Privacy Act of 1974 GSU may release, at the discretion of the university, directory information pertaining to individual students without their written consent. However, the Act gives currently enrolled students the right to withhold the disclosure of directory information. Use this form if you do not want the university to release directory information.

Use this form if you are an undeclared student and you have not registered for more than 3 consecutive semesters (but not exceeding 15 consecutive semesters). You cannot use this form if you have already earned 12 hours as an undergrad undeclared student or 6 hours as a graduate undeclared student, or you are on academic suspension.
